JetPass LLC Assumption of Risk, Waiver, and Release of Liability

By enrolling in JetPass LLC and using any Partner Facility, I acknowledge and agree:

1. Voluntary Participation

I voluntarily choose to participate in physical exercise, training, and use of gym equipment.

2. Assumption of Risk

I understand risks include but are not limited to:

• Muscle strains

• Sprains

• Slips and falls

• Equipment malfunction

• Heart events

• Illness exposure

• Serious bodily injury

• Permanent disability

• Death

I knowingly assume all risks, known and unknown.

3. Medical Responsibility

I certify I am physically able to participate or accept all responsibility for doing so.

4. Release of Liability

To the fullest extent permitted by law, I release and discharge JetPass LLC, its owners, officers, employees, contractors, affiliates, and agents from any and all claims arising from:

• Personal injury

• Property damage

• Illness

• Negligence of third parties

• Death

• Use of Partner Facilities

5. Third-Party Facilities

I understand gyms are independently owned. JetPass LLC does not inspect, supervise, or control these facilities.

6. Indemnification

I agree to defend and indemnify JetPass LLC against claims brought by me or others arising from my participation.

7. Binding Agreement

This agreement binds me, my heirs, estate, assigns, and legal representatives.

8. Severability

If any provision is unenforceable, the remainder remains effective.

9. Electronic Acceptance

Checking a box or digitally signing up via jetpasscrew.com constitutes legal signature.
